When to Consider Dermal Fillers

One of the first signs of aging is seen in our faces. With aging, the skin of the face loses elasticity and has decreased cellular turnover. This results in sallow and sunken features that are prone to wrinkles and creases. In addition, expressions of laughing, smiling, and frowning can leave deep creases in the skin. The loss of natural fullness in the cheeks and lips can leave the face looking aged.

Dermal fillers restore lost facial volume, decrease wrinkles and reduce the appearance of fine lines by filling and plumping needed areas. Dermal fillers can provide age restoring outcomes without dramatically changing the appearance of the face.

Expected outcomes of a dermal filler depend on the area to be treated. In general, dermal fillers are used to “fill” needed areas on the face. These can include mild to severe wrinkles, creases and lines. In addition, sallow or sunken regions of the face and cheeks as well as for plumping thin lips.

Common treatment areas include:

  • Smile lines: Known as nasolabial folds, these run from the sides of the nose to the corners of the mouth. They are a result of lost facial volume and lost elasticity of the skin.
  • Marionette lines: These lines run straight down the corners of the mouth, making you look unhappy or as if you are frowning, when you truly are not. They form as facial volume depletes and deep creases form.
  • Corners of the mouth: The corners of the mouth can also turn downward with age, with lost volume below the mouth.
  • Pre-jowl fold: This region of the face is the area on either side of the chin. Facial volume loss in this region results in sagging skin and a hollow, sunken appearance.
  • Chin wrinkles: These form horizontally across the chin and can take away from a naturally beautiful smile. They are a result of facial volume loss and can appear gradually with time.

There are many dermal filler products and we are pleased to carry a wide assortment for optimal outcomes and patient specificity.

Brands of Dermal Filler

Juvederm

Juvederm injectable filler allows you to reduce fine lines and restore lost facial volume without altering your ability to achieve natural facial expressions. The candidate for Juvederm is looking to reduce or eliminate* the appearance of mild to severe facial lines and wrinkles, while maintaining the ability to achieve natural facial expressions.

Juvederm is an injectable gel made of hyaluronic acid. Hyaluronic acid is a complex sugar that is found naturally in the human body. It works to hydrate the skin, providing volume. With age, our bodies have a difficult time producing hyaluronic acid, which leads to facial wrinkles and lost facial fullness.

The areas most often treated are the lines around the nose and lips. In addition, Juvederm can be used to restore lost volume to the cheeks and lips. It is often used to smooth the lines around the mouth and plump the lips. Juvederm is FDA approved for safety and effectiveness in people of all skin tones.

  • Juvederm XC gel is used for injection into the facial tissues, correcting moderate to severe facial folds. Creases found between the nose and lips are excellent areas to target.
  • Juvederm Ultra XC is meant for injection into the lips and areas around the lips, augmenting the shape and creating greater fullness.
  • Juvederm VOLUMA XC gel is meant for deep injection in the cheeks to correct for age related volume loss, restoring a more youthful and full appearance.

Perlane

Perlane is another hyaluronic acid based dermal filler. Hyaluronic acid is a naturally occurring sugar that hydrates and plumps the skin. This dermal filler is intended to fill deeper facial wrinkles and folds, such as those around the nose and mouth and frown lines.

Perlane gel is thicker than other dermal fillers, with larger particles. This means that the injection delivers more hyaluronic acid and is better suited for deep facial wrinkles.

Perlane is also used for plumping regions around the eyebrows, enhancing lips, defining a cupid’s bow and enhancing the contours of the cheeks and chin. It has also been used to reduce the appearance of scars and facial depressions from previous acne.

Radiesse

Radiesse is a dermal filler that is intended to smooth moderate to severe facial lines, such as those that extend from the corner of the nose to the mouth.

Immediately after injection, the filler adds volume under the skin and with time, Radiesse naturally stimulates the body’s collagen production. The results have been shown to last one year or more in many clients, which serves to produce immediate and longer lasting results.

Radiesse is made of calcium hydroxylapatite (CaHA) microspheres which are suspended in an aqueous gel carrier. Once injected, the substance gives immediate volume and also stimulates the body’s natural collagen production. The gel is absorbed and metabolized over time, leaving behind only the body’s natural collagen. With Radiesse, the results start out great and continue to get better with time.

Silk

Silk is another hyaluronic-acid based injectable filler that is meant for plumping and filling wrinkles around the mouth as well as filling, plumping and defining the lips.

Silk contains lidocaine, which is an anesthetic that reduces the discomfort of the actual injection procedure. This works to improve patient comfort during the injection.

The lips and lines around the mouth, show aging signs quickly, especially for individuals who may have smoked cigarettes. This results in thin lips, lost lip shape and increased vertical lines above the lip.

Silk is made specifically to provide natural results in the are of the lips and mouth, as it uses smaller, smoother injection particles in comparison to other products. The smaller size allows for precise injection into delicate areas.

What to Expect

If considering a dermal injection, the first step is to meet for an individualized consultation, where you will discuss your desires and expected outcomes. The clinical team at Ennis Plastic Surgery will help you determine which type of dermal filler will best suite your needs, as well as review any precautions. If you are on certain medications, you may need to stop taking them before injection, due to a risk of bleeding. For many individuals, there is little preparation needed before undergoing a dermal injection.

The area will be slightly numbed with a local anesthetic to reduce discomfort. For most injections, the entire process takes 20-40 minutes, depending on how many injection sites are administered. The clinician determines the most appropriate injection sites and then injects the filler accordingly. There is typically very little recovery needed after an injection. There may be some mild redness or swelling in the area with mild discomfort. Typically these side effects are alleviated within a few days.

Many patients achieve ultimate outcomes by combining different fillers for specific needs. For example, an individual may plump facial contours with fillers, soften wrinkles with injectables, and plump the lips with Juvederm.

Difference between Relaxers and Fillers

Oftentimes, people are confused between fillers and relaxers. Dermal fillers and relaxers can generate some similar outcomes, reducing fine lines and wrinkles and restoring a more youthful appearance. However, their mechanisms of action are much different.

  1. Dermal fillers work by plumping wrinkles and concavities in the face. They lift areas of sagging skin and volumize the lips.
  2. Relaxers work by relaxing the muscles that cause lines and wrinkles. They temporarily block muscles from contracting, impeding their ability to cause lines in the first place.

Depending on your area of concern, the clinicians at Ennis Plastic Surgery will be able to direct you to the best treatment option. Some individuals have excellent outcomes by utilizing both fillers and relaxers for a treatment we call the Liquid Facelift.
